Common law declaration
Declaration of common law status is either a standard form document or a declaration. It may be required in cases like your CRA (tax) declaration, pension, immigration, employment, benefits or support applications.
It is generally a written statement made under oath or affirmation confirming the status of the relationship. Information to be mentioned includes names, contact information, addresses, dates of birth and dates of commencement and ending the relationship.
The document is completed by you subscribing the date and signing in the presence of a commissioner of oaths or a notary public who signs under his seal and stamps.
